Commentary on Ohtahara

چکیده
Dr. Ohtahara’s discussion addressed the results of a postmarketing study of adverse events reported by Japanese patients treated with zonisamide monotherapy and polytherapy. The 1512 study participants included both adults and children with partial and generalized seizures and no previous zonisamide treatment. Adverse effects were reported and categorized by each participating physician and ranked, according to a predetermined scale. The adverse effects and teratogenicity were compared with the results from the zonisamide developmental trial. Because of the benzisoxazole-based chemical structure of zonisamide, adverse effects similar to other currently used antiepileptic drugs may be expected. However, Dr. Ohtahara did not anticipate the adverse effects of zonisamide therapy to be more severe than those noted in other antiepilepsy drugs. This expectation was supported in the postmarketing study in which 476 (31.5%) participants reported 1089 adverse effects. This rate was lower than the 51.3% adverse effect rate reported in the premarketing trial. The most frequently reported adverse effects in Dr. Ohtahara’s study included psychiatric, gastrointestinal, central and peripheral neurological symptoms, and hypohidrosis. However, it is the documented cases of teratogenicity that stand out as warranting concern and

Substantial and sustained seizure reduction with ketogenic diet in a patient with Ohtahara syndrome
Ketogenic diet has been shown to be efficacious in some epileptic encephalopathies but rarely reported as being useful in children with Ohtahara syndrome. This could possibly be attributed to the rarity of the disease and associated short survival period. We report on a 5-year-old child with Ohtahara syndrome, whose seizures failed to improve with all known medications, continued to show persis...

STXBP1 HAPLOINSUFFICIENCY IN EARLY ONSET EPILEPTIC ENCEPHALOPATHY Early onset epileptic encephalopathies that occur in very early childhood are rare but particularly catastrophic forms of epilepsy that are invariably associated with significant neurological morbidity (Nordli, 2012). Mutations in the Syntaxin Binding Protein 1 (STXBP1) gene have been linked with two distinct but related forms of...
متن کاملEarly-onset epileptic encephalopathies: Ohtahara syndrome and early myoclonic encephalopathy.
Ohtahara syndrome and early myoclonic encephalopathy are the earliest presenting of the epileptic encephalopathies. They are typically distinguished from each other according to specific clinical and etiologic criteria. Nonetheless, considerable overlap exists between the two syndromes in terms of clinical presentation, prognosis, and electroencephalographic signature. Newer understandings of u...

متن کاملImpact of early hemispherotomy in a case of Ohtahara syndrome with left parieto-occipital megalencephaly
This report illustrates the usefulness and safety of very early hemispherotomy in an infant with Ohtahara syndrome (OS) secondary to left parieto-occipital megalencephaly. It provides evidence that surgical intervention might provide promising results in selected cases, and that young age is not a contraindication for this type of surgery.
